on the changing the patch for me..in a sec i will put on the new one..i do feel much better today..falling asleep on the couch 2 nights straight was a big hurt.  i had a great sleep for 6 hours this morning in my bed.Smile and probably was bad the other night because i walked to much that day..thinking about getting one of those small folding chair cane things..a cane that turns into a small seat..sometimes i just have to sit down or fall down.

yep

thanks...when i said sit or fall i want to make it clear i lose my balance/left leg buckles or i just tilt over..not under heavy influence or anythingEmbarassed it does look that way though.Laughing

That's no good at all. I had a family member who would lose the strength in their leg out of nowhere and would just go down if she did not have something to grab on to. I'm sure you did not take offense, but I didn't mean to laugh at you all the same. How awful that must be for you, I thought you falling was a comedic exaggeration since you are good at those subtle jokes. I have weakness in my right leg, but thankfully no where near completely falling yet, just tripping. I go to step and it is like there is no leg there I guess you could say. How did your weakness start out?

my bigger problem then the pain

after 3 back operations in 3 days, the left leg was totally useless..I used a walker for maybe 2 months.  If i am walking or standing and my weight is on that left leg at times i just tip over or fall.last year i broke my left elbow falling and  within a month of getting that cast off i  fell and broke a bone in the left handYell...my arach gets to both legs.

Right now i am in a hard spasm in my thighs  and calfs//just plain  stiff     lower back is killing me..i hate everything being tied together    Yell

Reservoir Patches

I've been using the Watson because they were the least expensive (25mcg/hr is $59; 75 is $114; and 100 is $118), but I had a red ring on my skin outlining the last 100 I took off (the reservoir type tend to be larger and it actually cut into my skin when I would bend over).  I have a High Deductible Health Plan so I pay the first $3,000 of Dr. visits & Meds.  Now, that I've satisfied that deductible, I'll probably switch to the matrix type.  I hope they work as well.  Today, I bought 10 Watson 100's for $137 and 10 Mylan 25's for $135.  The Mylan 25's are much smaller, seem to stick better and are more comfortable than the Watson 25's.  They are more flexible too which might be the reason why they stick better.

A few notes about the Watson's: Except for perhaps some minor temperature sensitivity, the dose rate is what it says on the label, such as 100micro grams per hour.  Since the amount of Fentanyl in the Watson 100mcg/hr size is listed as being 10 mg, the patch will deliver medication for 100 hours. This might done in case you miss/forget the 72 hour mark or get the day number mixed up and don't switch for 4 days (96 hours).

The serum level peaks about 36 hours after you first put it on.

You will have 50% serum level 17 hours after you take it off or it runs out of medication.

Based on the above, for those of you in pain only two days after putting it on probably need a higher dose rather than putting a new one on.

I currently use 1-100 + 1-25 because it's usually cheaper than 1-75 + 1-50 but now that all I'll pay is the co-pay, might use the latter combination for comfort.

I had a thought to share about whether this is true or not.  Since, we are talking about a gel patch (Watson), there is indeed 10.0 total mg of Fentanyl in the Watson 100 mcg/hr Fentanyl patches.  Assuming the patch should last 4 days is not an unreasonable thought.  The problem with that is, as there is less and less gel left in the patch, the membrane that meters the drug release is no longer completely covered with gel, and that causes there to be unreliable / unpredictable drug release (usually less than normal).  The matrix style patches, like the Mylans, do not have this problem (if I remember correctly).

Gel Patch vs Matrix

Good point.  I didn't think about the entire membrane not being covered.  I think that would mean a lower dose rate over time for many people but I find myself always rubbing the patch which would move the gel all over the membrane and perhaps a more consistent dosage for me.

According to Wikipedia, the reservoir/gel type of Duragesic patches were to be discontinued in the US as of July 2009.  If correct the two boxes I bought today are over a year old.  I guess that's OK since they both have a 2012 expiration date.

Also, that would mean we will all eventually be using the matrix type which, based on other posts above, seem to have inconsistent dose rates.  I hope that gets fixed.

Another minor problem with one brand of the matrix type is its tan color that rubbed off onto my shirts.

Have you tried putting a cover patch over the top of it to prevent the ink from being rubbed off on your clothes?

Color Bleed

Perhaps I misunderstood the directions that come with the patches that state if the patch is loose, you can tape the edges.  I interpret that as you should not put tape over the any part of the patch except the edges.  Is that incorrect?
